Who Provides The Services?

The Hospice Team is made up of every person concerned with the care and comfort of the patient. The patient and their family are at the center of the team joined by the patient’s physician and clergy. Hospice nurses, pharmacists, home health aides, social workers, physicians, counselors, dieticians, occupational therapists, physical therapists and speech therapists make up the professional team and a group of trained volunteers help to complete the holistic approach to providing care.

  • Around the clock response to problems and questions with 24-hour on-call services by professional staff
  • Pain and Symptom Management including medications, skin care, continuous care during crisis, IV therapy and non-pharmaceutical comfort measures
  • Physician’s services
  • Social Worker services
  • Medical equipment and supplies
  • supportive counseling for patinet and family members
  • Spiritual Care / Chaplain services
  • Nutrition and Dietary services

How Are Patients Referred?

Anyone can refer a patient with a life-limiting condition, who seeks comfort and quality of life, rather than curative treatment measures.  The patient’s doctor and the hospice team use their best clinical judgement to determine if the patient is terminally ill with a life expectancy of six months or less, ishould the disease run its normal course.
